Survival after Treatment of Malignant Fibrous Histiocytoma of Bone.

Abstract
Purpose : Malignant fibrous histiocytoma of bone is relatively rare tumor and there aer some difficulties in its diagnosis and treatment. Its prognosis is somewhat unpredictable also. We reviewed our cases of malignant fibrous histiocytomas of bone to know their oncologic results and more effective treatment.

Materials and Methods : Sixteen patients with malignant fibrous histiocytomas in bone were registered in Korea Cancer Center Hospital from Dec. 1986 to Nov. 1994. Among them 14 patients had full medical information and were analysed with respect to treament, local recurrence and survival.Nice cases were male and 5 female. Average age was 34.9 years(range 17 to 65).

Location of the tumor was as follows : five cases in distal femur, 4 in pelvis and 1 distal tibia, proximal femur, proximal humerus, distal fibula, calcaneus each other. According to the Enneking`s classification, stage IIb was 12 and stage IIIb 2 cases. Nine patients took operations and adjuvant chemotherapy and/or radiation therapy, 3 operation only and 2 adjuvant chemotherapy and/or radiation therapy.

Results : Average follow up period was 42.4 months(6 to 113). Five cases were continuous disease free, 1 alive with disease and 8 died of disease at final follow up. Local recurrenece rate after operation was 30.0%. Kaplan-Meier`s actuarial 5-year continuous disease free survival rate was 50.0%.

Conclusions : Surgical margin was important in local recurrence and considered to have influence on the metastasis and ultimate survival. To improve the patient`s survival multimodality approach is necessary, especially more effective and intensive chemotherapy.
